BEFORE YOU STAND IN LINE....

PK and KINDERGARTEN PARENTS

Please bring with you a current immunization record.  Current means your child has had a 4 year old checkup with all immunization series completed.


JUNIOR HIGH PARENTS

Immunization requirements were changed two years ago.  You will need to bring with you to registration:
a current immunization record-this will include a new Tdap, a 2nd chickenpox, 2 doses of MMR and 1 Meningococcal 
To register your student for school in August 2011, a current immunization record must be presented.

HIGH SCHOOL PARENTS

Your student will need a current Tdap or Tetanus booster IF it has been 10 years since their last DTaP.
