A physician's statement must be on file with Food Services before substitutions can be made for your child's meal. Please contact HISD Food Services at extension 5552.

ASTHMA

Have your physician complete and sign the asthma action plan.  This plan must be updated annually.

EPIPENS

Please have your doctor complete the Allergy care plan that includes orders for emergency response.  Instruction for self-administration of medication are also on this form.

SELF CARRY MEDICATIONS

Your physician must complete the Self-Carry/Administration that is included on the Asthma Care Plan and the Allergy Care Plan for your student to carry their asthma inhaler or EpiPen. 





Irresponsible behavior will result in the loss of privilege of self-administration of medication.
